PHP email problem form

Im using a php code for email to be sent. It works and are able to receive the email. The problem I am having is that on the contact page the person fills out the form and hits submit. It then takes you to another contact form and needs to be filled out again and hit submit. The email does gets sent and is received. This is the php code I am using. Email:

Thank you for your question on your php form. We are unable to provide specific coding solutions, but I would suggest simply trying to output the variables you are expecting to the screen to see if they are staying empty the first time through.
