Can you make a form for my page?
I need a simple form to have info sent to my email address but I'm having difficulty getting it to work on your server. I don't have experience with CGI.
It should look like this:
First Name
Last Name
Phone
Check Box 1: Red Maple Acer
Check Box 2: Chinese Pistache
Check Box 3: Raywood Ash
Submit to******@*********.com
