how to connect single page to database with php.

I don't use the comments form for two page ,but i use only on my contact page to comments for whole website ,but here a problem when i comments from this form it gives me an error as:
"invalid id "
Plz....! tell me the solution of this problem.

Please try again later. I noticed the URL when you get the error is: http://neq3.com/errors/404.html?applicationsforall.com I recommend setting the form up with this service (neq3), or following our guide on how to create a Form.
